Morning Health Talk

New Delhi, April 28, 2018:

Dear Colleague

In the field of Clinical Medicine Medall Healthcare has announced the launch of SASH – ‘Stay Aware. Stay Healthy’ health screening package of 57 non-fasting tests done @ 640 Rs.

More US children are considered to have elevated blood pressure ( 14.2% from 11.8%) under new American Academy of Pediatrics guidelines published in JAMA Pediatrics. The 2017 BP guidelines made 2 important changes, first was the exclusion of children who are overweight or obese from the charts used to define normal BP because they tend to have higher BP and worse cardiovascular outcomes later in life. Also, diagnostic thresholds were revised to better align with adult definitions. Based on the AAP guidelines, the estimated population prevalence of elevated BP increased from 11.8% to 14.2%.

Continuing update in AMR is important for us: First is misuse of colistin. Over 95% of the import of colistin for growth promotional use is from China. China banned the growth promotional use of colistin last year. We must fight for its banning of its promotional use. In humans the distribution of colistin to the pleural cavity, lung parenchyma, bones, and cerebrospinal fluid is poor. Therefore, consider other routes of administration like inhaled and intrathecal administration.

The following bacteria should make us worry. Get updated about all of them.

Priority 1: CRITICAL: Acinetobacter baumannii, carbapenem-resistant; Pseudomonas aeruginosa, carbapenem-resistant and Enterobacteriaceae, carbapenem-resistant, ESBL-producing

Priority 2: HIGH: Enterococcus faecium, vancomycin-resistant; Staphylococcus aureus, methicillin-resistant, vancomycin-intermediate and resistant; Helicobacter pylori, clarithromycin-resistant; Campylobacter spp., fluoroquinolone-resistant; Salmonellae, fluoroquinolone-resistant and Neisseria gonorrhoeae, cephalosporin-resistant, fluoroquinolone-resistant

Priority 3: MEDIUM: Streptococcus pneumoniae, penicillin-non-susceptible; Haemophilus influenzae, ampicillin-resistant and Shigella spp., fluoroquinolone-resistant.

All AMR cases should be made notifiable. Public Health England recorded 36 cases of pan-resistant-infections between April 2013 and February 2018, this includes the patient who picked up “super-gonorrhoea” in south east Asia which was resistant to the two most commonly used antibiotics.  Unless each case is notified and acted the problem will not be over.

Weather: Temperature recoded at 1430 hours have risen by 3-5°C at one or two pockets of Jammu & Kashmir and Rayalaseema; by 1-3°C at some parts of East Madhya Pradesh and Vidarbha and at one or two pockets of Punjab, Himachal Pradesh, Uttarakhand, East Rajasthan, West Madhya Pradesh, Jharkhand, Chhattisgarh, Odisha, Andhra Pradesh, Coastal Karnataka, South Interior Karnataka and Kerala

Legal

In Haryana 3 doctors at the Haryana Civil Hospital were suspended in connection with the death of a newborn baby after a woman was forced to deliver the child in an ambulance without medical aid.

Delhi Medical Council took suo moto cognizance of a case, wherein a man’s leg was wrongly operated upon at Sushruta Trauma Centre, while he was admitted with a head injury.

The Allahabad High Court ruled there is no evidence of medical negligence against Dr Kafeel Khan while granting him bail nearly eight months after he was arrested following the death of over 60 children at Gorakhpur’s BRD Medical College Hospital.

“It is our bounden duty and obligation of the civil society to ensure that the medical professionals are not unnecessary harassed or humiliated so that they can perform their professional duties without fear and apprehension.” (Kusum Sharma & Others versus Batra Hospital & Medical Research Centre, 2010 (3) SCC 480)

Quantum Physics is the future: As per Aashish Clerk, a professor at the Institute for Molecular Engineering at the University of Chicago ““In our system, we have two very small vibrating drumheads. If you just look at one of the drumheads, its motion would appear to be completely random. However, if you look at both, you would see that the motion of the two drumheads are extremely correlated: when one drumhead moves up, the other moves down” This correlation is too strong to be understood using classical physics. Their approach led to states of entanglement which lasted for long periods of time, sometimes up to half an hour.

In another study scientists forged quantum connections between separate regions within clouds of ultracold atoms, demonstrating entanglement between thousands of particles in two different locations. Previous similar experiments had entangled several thousand atoms, but only within one entire cloud (SN Online: 3/25/15).

Entanglement is a bizarre quantum phenomenon where separate objects, whether individual particles or groups of particles, behave as a conjoined entity. Measuring one object immediately reveals information about the other, even when the two are in different locations, such as distinct atomic clouds.
