Paras Health Gurugram first in the country to implant a second capsule-sized pacemaker in 88-yr-old man

In a groundbreaking medical achievement, doctors at Paras Health Gurugram have successfully implanted a second Micra pacemaker in the heart of an 88-year-old patient. This remarkable feat marks the first time in India that the world’s smallest pacemaker, known as Micra, has been implanted in the same heart for a second time. The innovative
procedure was necessitated by the patient’s severe lightheadedness and loss of consciousness due to the depletion of the battery in the first Micra pacemaker.

The patient presented with symptoms of lightheadedness and uneasiness at Paras Health Gurugram. An ECG revealed a critically low heart rate of 30 beats per minute, prompting immediate admission to the hospital. Initial treatment involved the insertion of a temporary pacing catheter, a special wire with a balloon designed to prevent heart perforation. This was followed by the intricate task of implanting the second Micra pacemaker above the existing one,
ensuring no interaction between the two devices.

This is an index case and has never been done in the country before," said Dr. Amit Bhushan Sharma, Director And Unit Head – Cardiology – Interventional, Paras Health, Gurugram, Director And Unit Head – Cardiology – Interventional, Paras Health, Gurugram.The Micra is the world’s smallest pacemaker, about the size of a vitamin capsule, and is leadless. When the patient came in with battery depletion and loss of consciousness, we had to act swiftly. We successfully implanted the second Micra pacemaker on top of the first one without stopping his
blood thinners. This innovative approach minimized the risk of heart perforation and ensured both pacemakers functioned independently," he added.

Seven years ago, in 2017, Paras Hospital was among the first in India to implant the Micra pacemaker, and the same patient had received his initial device during that time. The recent procedure underscores the hospital's ongoing commitment to pioneering advanced cardiac treatments and setting new benchmarks in patient care.

The procedure, which took approximately two hours, was carefully monitored using special imaging and echo guidance to position the second pacemaker accurately. We used a basic type of catheter with a balloon to avoid heart perforation. The second pacemaker was placed above the first to reduce the risk of complications. Special imaging
ensured that both pacemakers were correctly positioned and did not interact with each other, Dr. Amit Bhushan Sharma, Director And Unit Head – Cardiology – Interventional, Paras Health, Gurugram added.

The patient had previously undergone angioplasty in 2014 and had been managing well with the first Micra pacemaker until the recent battery depletion. His condition deteriorated as he had not been getting regular check-ups or follow-ups. Following the successful implantation of the second Micra pacemaker, the patient was closely monitored and discharged after 48 hours. He is now doing well and living a normal life.
