9-year-old boy from Kolkata undergoes successful liver transplant for rare alagille inherited disorder at Indraprastha Apollo Hospitals

In Alagille syndrome, liver damage is caused by abnormalities in the bile ducts. These ducts carry bile from the liver to the gallbladder and small intestine. In Alagille syndrome, the bile ducts may be narrow, malformed, and reduced in number (bile duct paucity). As a result, bile builds up in the liver and causes scarring, cirrhosis and liver failure. Rajdeep initially developed jaundice along with itching. While the itching was manageable in the beginning, it deteriorated and affected the sleep cycle of Rajdeep. At the age of one, he was admitted to Indraprastha Apollo Hospitals in 2014 and a diagnosis of Alagille syndrome was made.
Upon further tests, he was diagnosed with alagille syndrome, which can further lead to cirrhosis and liver failure. For the past few years, Rajdeep was being treated with medications and was in constant observation virtually. With no respite in his condition, at the age of 8, Rajdeep developed liver failure and was recommended an immediate liver transplant. Additionally, 54 – year old Shameen, who was suffering from chronic liver disease was waiting for over a year for the transplant. He required a transplant as his liver had deteriorated and he was developing ascites, swelling in legs, repeated episodes of mental confusion (encephalopathy) and also kidney problems
The liver transplant team consisting of Dr Anupam Sibal, Group Medical Director, Senior Paediatric Gastroenterologist, Dr Neerav Goyal, Senior Consultant, Liver Transplant including Dr Arun V, Consultant – Liver Transplant Unit, Dr Smita Malhotra, Consultant, Pediatric Gastroenterologist, Indraprastha Apollo Hospitals, New Delhi, Dr Nameet Jerath, Senior Consultant, Pediatric Pulmonology and Critical Care, Indraprastha Apollo Hospitals, New Delhi, Dr Varun M, Consultant, Liver Transplant Unit, Dr H K Garg , Senior Consultant, Anesthesia, Indraprastha Apollo Hospitals, New Delhi, was also involved in the operation and Dr Pradeep K, Consultant, Liver Transplant Unit arranged for the liver and Rajdeep was brought to Delhi on 17th November 2022 for the liver transplant.
Dr Anupam Sibal, Group Medical Director, Senior Paediatric Gastroenterologist, Apollo Hospitals Group,said,” Alagille syndrome, is a rare genetic disorder, seen in 1 in 30,000 children and can affect multiple organ systems of the body including the liver, heart, kidney etc. Rajdeep’s condition had deteriorated with deepening jaundice and severe malnutrition. He needed a liver transplant on survival as there was no suitable donor in the family.
On 16th November 2022, a brave and kind family decided to donate the organs of their child who was declared brain dead after a tragic accident. Rajdeep had been waiting for a cadaver liver donor. After necessary approvals from NOTTO we decided to harvest the cadaver donor’s liver. It was a healthy liver hence we decided to split the liver into two halves. As Rajdeep weighed only 17 kgs at 9 years, he would get the left lobe and a 54-year-old patient got the right lobe. We gave a fit to fly certificate at 11pm and got Rajdeep and his family on a flight from Kolkata at 11:45 pm. Rajdeep arrived in Delhi on 17th November at 2am and on 17th November at 2pm, he was wheeled into the OT for a cadaveric liver transplant.”
Dr Neerav Goyal, Senior Consultant, Liver Transplant, Indraprastha Apollo Hospitals, said, “While 9-year-old Rajdeep was waiting for a liver transplant since a year, 54-year-old Shameen was suffering from chronic liver disease since several years. For the past two years, Shameen has been admitted to the hospital several times as his liver had deteriorated and he was developing ascites, swelling in legs, repeated episodes of mental confusion (encephalopathy) and also kidney problems. Three months before the transplant he was also diagnosed with tuberculosis of the lungs from prolonged illness and low immunity. We are happy that because of the noble act of cadaver organ donation we were able to save two lives.”
