Narayana Health SRCC Children’s Hospital, Mumbai, has successfully treated a rare and complex case of Congenital Diaphragmatic Hernia (CDH) in a 20-month-old baby girl, who was brought to the hospital in critical condition. She had been initially operated upon at another facility without relief.
The child, who was experiencing severe respiratory distress and in need of continuous oxygen support, was rushed to the Emergency Department at Narayana Health SRCC Children’s Hospital by ambulance. Under the care of Dr. Rasik Shah, Senior Consultant, Paediatric Surgery, an immediate evaluation including CT angiography and routine blood tests was conducted.
Previous surgery performed in a smaller city had failed to resolve the issue, and the child’s condition had further deteriorated. Upon re-evaluation at Narayana Health SRCC Children’s Hospital, Mumbai, imaging revealed that a portion of the liver had large herniated into the chest cavity, causing complications. Attempts to reposition the liver into the abdomen were unsuccessful due to suspected kinking of the hepatic vein (where the blood vessel is twisted or bent abnormally) and inferior vena cava (IVC), which affected venous return.
A multidisciplinary team, including Dr. Ashok Thorat (Senior Consultant—Liver Transplant and HPB Surgery), Dr. Soonu Udani (Medical Director & Head of Critical Care & Emergency Services), and Dr. Aoyon Sengupta (Senior Consultant— Critical Care Medicine), Dr. Pradip Kaushik (Senior Consultant— Paediatric Cardiac Surgery) and Dr. Nandini Dave (Senior Consultant— Paediatric Anesthesiology) convened to devise a detailed treatment plan. It was decided to proceed with a second surgery using a thoraco-abdominal approach, with a possible liver resection and diaphragm repair.
The surgery was successfully performed, and the diaphragm was reinforced with mesh. The child was shifted to the Paediatric Intensive Care Unit (PICU) on ventilator support and extubated after three days. Over the next week, she gradually weaned off oxygen and resumed full feeds.
After a two-week hospital stay post-surgery, the patient was discharged in stable condition, without the need for supplemental oxygen. At her follow-up 45 days later, she had gained 600 grams and was showing signs of healthy recovery.
