Three live laparoscopic surgeries were performed in just 4 hours

Doctors at HCMCT Manipal Hospital, Dwarka (A multi-specialty facility) successfully performed three live laparoscopic surgeries led by Dr. Randeep Wadhawan, HOD and Consultant – Department of Minimal Access Surgery, Bariatric & Metabolic Surgery & Gastrointestinal Surgery and his team of doctors. The live surgeries were performed at the conference on Fellowship in Advanced Laparoscopic Surgery (FALS UGI 2021), organised by HCMCT Manipal Hospital, Dwarka at Vivanta by Taj Dwarka, New Delhi. The patients underwent live procedures, like para-oesophageal hernias type III, metabolism and bariatrics.

Among the three surgeries, the 43-year-old patient from Mumbai came with a complaint of abdominal pain, regurgitation of food and vomiting on and off. On further investigation he was diagnosed with Para-oesophageal Hernia Type III. A para-oesophageal hernia occurs when the lower part of the stomach, or other organs move up into the chest. When the gastroesophageal (GE) junction, the stomach, or other abdominal organs such as the small intestine, colon, or spleen moves upward towards the chest where they don’t belong, the condition is known as para-oesophageal hernia. It occurs in 5% of population suffering from Hiatus hernia.

This was an unusual case as 60 per cent of his stomach went inside his chest (thoracic cavity), and there was an 8 cm defect in the diaphragm (cavity dividing thoracic cavity and abdomen) from which the stomach had entered his chest. As a result, his food began to travel in the upward direction, because of which he began to develop ulcers in his food pipe. Dr Randeep and his team used laparoscopic surgery to move his stomach down from his chest (thoracic cavity) and closed the hernia defect.

Dr. Randeep Wadhawan, HOD and Consultant – Department of Minimal Access Surgery, Bariatric & Metabolic Surgery & Gastrointestinal Surgery, HCMCT Manipal Hospitals, said, “The patient was diagnosed with a large para-oesophageal hernia type III, which is a rare case of hernia. We used laparoscopic surgery to move the stomach down and then used specific absorbable mesh to reinforce the defect. We also rebuilt the lower oesophageal sphincter to prevent reflux, and within 24 hours, the patient was able to drink liquids by mouth, eventually progressing to an oral diet. The patient is on the path to recovery and soon will be able to perform activities without any difficulty.”

Additionally, there were two procedures performed, one on a 44-year-old patient from Bihar suffering from diabetes and obesity. Before the surgery he weighed around 100kg, had a BMI of 42Kg/m2. He was suffering with Diabetes Type II and was suggested by his Endocrinologist to either start taking Insulin or consider Metabolic Surgery. The patient underwent a tailored Laparoscopic Roux-en-Y gastric bypass, a form of bypass surgery used to treat obesity and diabetes patients. Post the surgery, the patient will continue to lose weight and, will be able to achieve a normal BMI in around 9 months and will also be diabetic free.

The third surgery was performed on a 59-year-old super obese woman with a BMI of 52Kg/m2 who came with an acute upper abdominal pain and was diagnosed with an acute inflammation of the gall bladder due to gall stones. She also had osteoarthritis of knees and had difficulty in walking.  She underwent Laparoscopic Sleeve Gastrectomy and laparoscopic gallbladder surgery at the same time. After 24 hours, the patient was doing fine.
